Prescription Topical

— Rx Required · Topical Chemotherapy

5-Fluorouracil (5-FU) cream selectively targets rapidly dividing atypical keratinocytes — causing their controlled destruction while largely sparing surrounding normal tissue. Treatment courses of 2–4 weeks produce a visible inflammatory response followed by clearance of the pre-malignant field.

Imiquimod activates the innate immune system to recognise and destroy atypical cells through a different pathway — particularly effective for superficial field treatment and low-risk basal cell carcinoma.

Both are highly effective and often combined with or preparatory to laser resurfacing for deeper field damage.

Photodynamic Therapy

— In-Clinic Rx · ALA + Light Activation

Photodynamic therapy applies aminolevulinic acid (ALA) or its methyl ester (MAL) — a photosensitiser preferentially absorbed by metabolically active pre-malignant cells. Activated by specific light wavelengths, it generates reactive oxygen species that selectively destroy those cells.

PDT treats the full damaged field simultaneously — both visible actinic keratoses and invisible surrounding pre-malignant tissue. One of the most effective field treatments in dermatology, with excellent cosmetic outcomes. Frequently combined with CO₂ laser pre-treatment to enhance photosensitiser penetration into deeper tissue layers.

Ablative Laser — Fractional CO₂

— Physician Only · Depth-Calibrated Field Reset

At sufficient depth, fractional CO₂ laser physically removes the entire atypical epidermal field — including cells too abnormal to respond to immune stimulation but not yet visibly malignant — and replaces it with a new epidermal population from healthier, deeper keratinocyte stem cells.

At ≥600 µm it simultaneously eliminates senescent cell SASP reservoirs, triggers deep collagen remodelling, and resets the biological age of the treated zone.

Skincare as Prevention — Published Evidence

Most skincare is reactive — addressing what has already happened.
